Supporting a Loved One After a Hospital Discharge

Coming home from hospital can be a vulnerable time. Learn how to prepare, what support is available, and how reablement care can aid recovery.

Planning for discharge

Hospital discharge planning should begin as early as possible. Speak to the ward team about your loved one's needs and ask for a discharge assessment. This will identify what support is required at home and whether any equipment or adaptations are needed.

If your loved one needs ongoing care, the hospital should not discharge them until appropriate arrangements are in place. You have the right to be involved in these decisions.

Reablement care

Reablement is a short-term, goal-focused approach to care that helps people regain independence after illness, injury, or a hospital stay. Rather than doing things for you, reablement carers work alongside you, encouraging and supporting you to rebuild confidence and skills.

Many local authorities offer free reablement care for up to six weeks following hospital discharge. After this period, a reassessment determines whether ongoing support is needed.
